Skilled Nursing Care

Is frequently required after an acute care hospitalization and/or surgical procedure. You will receive excellent skilled nursing care provided by a dynamic team including licensed nurses, state tested nursing assistants, registered dietitian with regular physician rounding.

These services include but not limited to:

  • Comprehensive person centered care plan and discharge planning
  • Cardiac care and stroke rehabilitation
  • Post-surgical and wound care
  • IV therapy
  • Tube feedings and ostomies
  • Dialysis care/In-house peritoneal dialysis
  • Bi-pap/C-pap
  • Tracheostomy care and suctioning
  • Diabetes management
  • Pain management
  • Dementia care
  • Dietary and nutrition management
  • Oxygen (up to 5 liters)
  • Palliative care
  • Hospice care
  • Laboratory testing
  • X-rays
  • Doppler testing

We offer complex wound care with our in-house certified wound care nurse.

Physicians/providers on-call 24 hours a day with physicians/providers in-house on a regular basis.

Our registered dietitian sees our residents on a regular basis to ensure they are receiving nutritional intake that supports healing.

We have a psychologist on staff to help our residents adjust to changes in their health status.
